Case Management Services Provides:
*Functional needs assessment , including evaluation of independent living and community skills, identification of needed supports and recommendations for services
*Life skills training, including social and recreational skills development
*Medical and rehabilitation treatment coordination
*Behavior Management
*Advocacy and Community Resource Identification
Who is eligible: Adults over 18 years of age with brain injuries
Funding: Private Insurance, Worker's Compensation, Brain Injury and Statewide Specialized Community Services (MRC), Private Pay
