CSA offers Residential, Case Management, Employment, Outreach and Recreational Services for adults with brain injuries on Cape Cod. Each section below can be expanded to learn about who is eligible, how it is funded and what services it provides.
Residential Services: CSA operates three group residences in Hyannis for adults with brain injuries. These homes provide long term supported living for adults with head injuries and other neurological impairments to maximize the quality of all aspects of their life. We also provide services to adults with brain injuries or other neurological impairments living in other group residences and individual apartments on Cape Cod. Services are provided to consumers on a as-needed basis with only as much supervision and assistance as necessary. After developing a custom, comprehensive plan of care addressing cognitive, functional, rehabilitation and social needs, we help consumers successfully take each step toward greater independence, while providing appropriate levels of quality care for significantly less cost than rehabilitation institutes or long term care facilities. These services can include, but are not limited to financial, organizational, medical, recreational, home care and maintenance, vocational and life skill training. With our comprehensive Needs Assessment, all areas of work, home and social life can be evaluated, and specific recommendations will be offered. Click to learn more about Case Management Services >>
